TAPPS Forms

TAPPS requires each athlete to have a physical examination each year prior to participating in a practice or competition. The physical consists of a Medical History Form and a Physical Form. The Physical Form must be completed by a physician.

In addition to the physical, TAPPS also requires both the athlete and the athlete's parents or legal guardians to complete and sign the Student Acknowledgment of Rules Form.

Both forms should be given directly to the athlete's coach or to the Athletic Administrative Assistant. Only one of each form is needed for the school year regardless of how many sports the athlete participates in.

If a high school student participated in a sport in either sub-varsity (JV) or varsity in the 9th, 10th, 11th, or 12th grade during the current school year, or the 9th, 10th or 11th grade during the previous school year at another school, the student must complete the Transfer Form. There is a deadline for submission for 2011-2012: September 7 for Fall Sports, December 7 for Winter Sports and February 22 for Spring Sports. The form requires signatures from the previous school, so it is important to complete the form as soon as possible upon acceptance. Instructions for completing the form are here.

School Forms

Each athlete and parent must read the Athletics Handbook. At the end of the Handbook is an affidavit confirming that the athlete and parents understand and will conform to the principles and rules outlined in the Handbook.

Each athlete and parent must also complete and sign a Participation Covenant for every sport he or she participates in. This form also should be given to the athlete's coach or to the Athletic Administrative Assistant.

At the beginning of each sport's season, the team coach will provide an Emergency Card which provides the coach with the necessary information to assist the athlete and parent in the event of a medical emergency.
